Pet Owner's Declaration

I am the owner of these pets OR I am authorised by the owner to sign this booking request form.
I agree on the arrival date lodged above to sign Petcare Extraordinaire's boarding contract. A proforma of the contract can be viewed here.
I recognise that all professional care will be given to my pet /pets and that I have noted in this form or discussed any concerns I may have with the Petcare Extraordinaire team member and I hereby release, discharge and indemnify Petcare Extraordinaire and any person or corporation associated with the boarding kennel / cattery, Petcare Extradinaire from all actions, lawsuits, demands, claims, causes of action and costs of every description whatsoever at law.
Whist Petcare Extraordinaire will take all take in keeping track of items that are brought in with your pets I understand that no responsibility will be accepted by Petcare Extraordinaire for loss of collars leads, chains, blankets, coats, toys, carriers, baskets etc.
I understand that occasionally illness may occur in an animal whilst boarding and understand that Petcare Extraordinaire accepts no responsibility for any illness occurring. In the event that I am not contactable by the provided numbers and /or nominated veterinary practice and it has been assessed by the Petcare Extraordinaire team that my animal’s condition has declined and/or requires veterinary attention, I understand Petcare Extraordinaire will seek veterinarian care provider who will perform any procedures deemed necessary and in the best interest of my animal. I hereby give permission for the administration of medication and IV fluids, diagnostic workup (blood work, urinalysis, x-rays) or procedures deemed necessary by your veterinary contact or the veterinary service we contact.
I accept all costs involved and will pay all fees owing at the time of departure unless alternative arrangements have been made with Petcare Extraordinaire in writing before signing this form (written agreement must be attached to this form for it to be valid).
I have read and understood this form. I understand that there might be a surcharge added to the bill for credit card transactions.
